Graceville Health Center's main campus is located at 115 West Second Street in Graceville, MN. Holy Trinity Hospital, Graceville Health Center Clinic, and Graceville Health Center Home Health are all located in the same building on the northeastern shore of Lake Toqua.
Grace Home is located at 116 West Second Street, across the street from the other facilities. To the southeast of Grace Home is Grace Village, the newest addition to the Graceville Health Center Campus.
Graceville Health Center is proud to offer a rural clinic in Chokio, MN, 13 miles east of Graceville on Highway 28. The Chokio Medical Center is located on Main Street Chokio.
Directions to Graceville Health Center Main Facilities:
From the south on Highway 75:
Turn left on County Road 18, follow this road for 1 mile. Graceville Health Center Clinic and Holy Trinity Hospital will be on your left, Grace Village and Grace Home will be on your right.
From the north on Highway 75:
Turn right onto Toqua Avenue, take the first left onto West Second Street. Follow this road for one block. Grace Home and Grace Village will be on your left, Holy Trinity Hospital and Graceville Health Center Clinic will be on your right.
