Graceville Health Center Home Health provides a variety of services in a person's own home in order to help patients maintain an independent lifestyle while receiving skilled personal care.
How can you obtain Home Health Services?
1. Referral to Graceville Health Center Home Health by a physician, self, family members, friends, or community agency. Anyone can refer.
2. Physician contacted by Home Health.
3. Home Evaluation of client and family by R.N.
4. Treatment Plan developed and authorized by a physician.

Nursing care provided by Registered Nurses:
An initial assessment in your home to determine needs.
Medication information and management. (Includes medication delivery and set-up, if needed)
Physical assessment: Blood pressures, pulse, temperature, blood sugars, etc.
Diabetic and dietary counseling and teaching.
Oncology care for cancer patients.
IV therapy, dressing changes and other skilled nursing procedures.
Blood draws for lab work.
Obstetric, pediatric and infant care.
Nail and foot care.
Assistance in obtaining medically necessary equipment.
Home Health Aides are available to assist with:
Bathing
Meal Preparation
Light Housekeeping
Dressing
